 Exhibitor's Commitment Form

Exhibitors may choose from a number of booth spaces as shown below.  Exhibit booths (B1-B14) inside Ballroom 5 are 8'x10'. The 6'x10' exhibit booths (H1-H11) outside Ballrooms 4 & 5  are spill over booths and will not be available until Exhibit booths (B1-B14) inside Ballroom 5 are filled.

 

 

 Each booth includes pipe/drape; 8’ skirted table; 2 chairs; wastebasket & a booth sign.

Spaces may be reserved in multiples to accommodate larger booths.

 

The cost of each exhibit booth (B1-B14) inside Ballroom 5 is $500.00 + one full registration ($550) for the First person. The cost of each spillover booth (H1-H11) will be posted after all the exhibit booths (B1-B14) inside Ballroom 5 are full.

 

Paid exhibit booths includes coffee breaks for a Second and Third Person. Access to technical sessions, a ticket to the banquet dinner and a copy of the proceedings are not included for the Second and Third Persons. Full Registration upgrades, CD-ROM or Banquet Ticket for the Second and Third Persons can be purchase with this form.

 

Persons registered using this form must not be authors or presenters, who must register using the Full Registration Form. Authors and Presenter are permitted to sever as exhibit booth staff at the forum and need not be listed here. However, one person, not an author or presenter must be registered with this form.

Minimum booth total  is $1050.00, which includes one full registration.

Full registration and booth are paid separately using the online payment or together by check.

 

Refunds for paid exhibit fees can be issued prior to January 1, 2009 with a $50 penalty.

 No refunds will be issued after January 1, 2009.

All refunds are subject to approval.
